In Diabetic Cats, Losing Weight Doubled the Odds of Coming Off Insulin
Based on a two-center randomized controlled trial published in the Journal of Veterinary Internal Medicine (March 2026), cross-checked against the study's University of Liverpool repository record and WSAVA global nutrition guidance.
A randomized trial put 72 overweight, insulin-treated diabetic cats on the same therapeutic diet and only changed the portion. Cats fed to lose weight came off insulin about twice as often as cats fed to hold steady, and even the ones who stayed on insulin ended up on a lower, steadier dose.
A rare kind of hope
Feline diabetes carries a kind of hope that most chronic illnesses don't. A diabetic dog needs insulin for life. A cat, caught early and managed well, can sometimes stop needing it altogether. Vets have known this for years. What they have not had is a controlled trial showing whether a structured weight-loss plan actually pushes more cats into remission, or whether that was a pattern clinicians only thought they saw. Now there is one.
The trial appeared in the Journal of Veterinary Internal Medicine in March 2026. Researchers at the University of Copenhagen and the Royal Veterinary College in London enrolled 72 overweight, insulin-treated diabetic cats across two clinical centers and split them at random into two groups. Every cat ate the same purpose-formulated diabetic diet, high in protein and low in digestible carbohydrate. What differed was the portion. Thirty-two cats were fed to lose about 2 percent of their body weight per week, an aggressive pace but a supervised one. The other 40 were fed to hold their existing weight steady on that same food. At 12 weeks, the researchers counted which cats no longer needed insulin.
The numbers
Sixteen of the 32 cats on the weight-loss plan reached remission, half the group. Of the 40 held at a steady weight, 12 came off insulin, a rate of 30 percent.
On identical food, the calorie cut made remission about twice as likely.
Few cats actually held the 2 percent-a-week pace for the full 12 weeks, which will surprise nobody who has tried to keep a human diet on schedule. The weight-loss group still lost an average of 7.2 percent of body weight over the study, against 2.7 percent in the control group. The calorie cut did its job even short of the number it was aiming at.
The cats that never reached remission were not left where they started. In the weight-loss group, their insulin dose fell by an average of 36 percent by the end of the study. Among non-remission cats in the control group, it went up by 28 percent. The same split showed up in glycemic variability, the swing between a cat's blood sugar highs and lows: down 45 percent in the weight-loss group's non-remission cats, down 7 percent in the control group. A cat that stayed on insulin was still better off for the weight it lost.
The portion, not the food
There is an obvious way to misread this, which is as a story about one commercial diet outperforming another. The trial does not show that. Every cat in it ate the same food. The only thing separating a 50 percent remission rate from a 30 percent one was how much of that food went into the bowl. It fits what clinicians treating feline diabetes have argued for years: obesity itself drives insulin resistance in cats, so taking the weight off addresses part of the cause and not only the symptom.
The food still mattered, though that part rests on other research rather than on this trial. A high-protein, low-carbohydrate therapeutic diet lets calories fall while protein stays high enough to protect lean muscle. In an earlier study of calorie-restricted overweight cats, both diet groups lost weight at the same rate, but only the cats eating the high-protein diet held on to lean mass. Serving less of an ordinary maintenance food is a different proposition: a 2026 analysis found that over-the-counter adult foods, once the portion is cut, more often fall below recommended amino acid and vitamin intakes than veterinary weight-loss diets do. In this trial, though, both groups ate the same food and only the amount differed.
Why this is not something to try at home
None of this is a reason to start cutting a diabetic cat's food on your own.
The weight loss in the trial was closely supervised. The target was about 2 percent of body weight per week, tracked and adjusted by researchers who could see each cat's insulin needs shifting in something close to real time. A cat losing faster than planned, or one that stops eating during a food change, is at real risk of hepatic lipidosis, a serious condition in which the liver is overwhelmed processing the body's own fat stores. That risk is why a diabetic cat's weight-loss plan belongs with a veterinarian and not with a slightly smaller scoop each week.
The word remission needs pinning down too. The researchers defined it as no longer needing insulin injections, assessed at the 12-week mark. It does not mean the underlying tendency toward diabetes has gone for good. A cat in remission can relapse, and is likelier to if the weight comes back, so the monitoring does not stop when the injections do. The trial's own results point the same way. The cats that never came off insulin still ended up on a lower dose with steadier blood sugar, and that is the kind of result somebody has to keep measuring to act on.
What to raise with a vet
The useful shift here is in how an owner thinks about a diabetic cat's weight. It is common to treat the insulin schedule as the whole of the treatment and the weight as background detail, something to mention at the next checkup. This trial puts the weight inside the treatment plan.
If a diabetic cat is also overweight, a few specific things are worth putting to a vet:
- whether a purpose-formulated diabetic or weight-management diet suits this particular cat
- what weekly rate of weight loss is safe given the cat's current condition
- how often the food amount should be rechecked as the weight comes down
- when the insulin dose needs reassessing, since the requirement shifts as weight comes off
